What is Support at Home?

Support at Home is the Australian Government's home care program that replaced the Home Care Package (HCP) scheme on 1 November 2025, established under the Aged Care Act.

It provides government-subsidised funding for older Australians to receive care services in their own home across three categories: Clinical Care (100% Government funded), Independence (co-contribution may apply), and Everyday Living (co-contribution may apply).

There are 8 funding classifications based on your assessed needs, with a maximum annual funding of approximately $78,000. Support at Home is managed through Services Australia and operates on a quarterly budgeting cycle, giving you flexibility to adjust services as your needs change throughout the year.

Who can access your services?

Our services are available to older Australians who have been assessed and approved for Support at Home funding through the Single Assessment System.

This includes three groups of participants:

Grandfathered Participants — transitioned from an existing Home Care Package on 1 November 2025.
Hybrid Participants — approved for HCP funding during the transition period but had not yet commenced services.
New Participants — assessed and approved from 1 November 2025 onwards under the new Support at Home classifications.

How are your services funded?

Under the Support at Home program, funding depends on the category of service you receive.

Clinical Care — including in-home nursing, wound care, medication management, chronic disease support, and palliative care — is 100% Government funded with no out-of-pocket cost to you.

For Independence services (allied health, physiotherapy, occupational therapy, assistive technology, home modifications) and Everyday Living services (personal care, domestic assistance, meal preparation, social support, community access, respite care), a co-contribution may apply based on your individual financial circumstances.

There is a lifetime non-clinical co-contribution cap of $130,000 to protect you from excessive costs. Your funding is allocated quarterly through Services Australia, and we work with you to plan your services within each quarterly budget to make the most of your funding allocation.

What does “Care Home On Wheels” mean?

Care Home On Wheels is GHCG's care delivery model that brings residential-quality care directly to your home.

Rather than moving into an aged care facility, you receive the full spectrum of professional care — clinical nursing, personal care, domestic support, and social connection — all delivered by a consistent, dedicated care team who know you by name.

The model is designed so you get the quality and comprehensiveness of a nursing home whilst remaining in the comfort and familiarity of your own home, surrounded by the people, routines, and environment you love. It means one provider, one team, and one care plan covering everything you need.

What services does GHCG provide?

We deliver services across all three Support at Home categories.

Clinical Care (100% Government funded) — in-home nursing by registered and enrolled nurses, medication administration and management, wound care, chronic disease management, health monitoring, palliative and end-of-life care, and post-hospital support.

Independence (co-contribution may apply) — allied health services, physiotherapy and exercise programs, occupational therapy, assistive technology, home modifications, and reablement and restorative care pathways.

Everyday Living (co-contribution may apply) — personal care, domestic assistance, meal preparation and nutrition support, social support and companionship, community access and transport, and respite care for carers.

We also offer two specialised short-term care pathways: Restorative Care (intensive rehabilitation following illness, injury, or hospitalisation) and End-of-Life (compassionate palliative care and support).

How do I get assessed for Support at Home?

To access Support at Home, you need to be assessed through the Single Assessment System using the Integrated Assessment Tool.

The process starts by contacting My Aged Care on 1800 200 422, who will arrange an assessment to determine your eligibility and the level of support you need. Based on your assessment, you will receive approval and be assigned a funding classification from 1 to 8.

You can contact My Aged Care directly, or speak with GHCG during a free consultation and we can guide you through every step of the assessment process.

Are your carers qualified and background checked?

Absolutely. All GHCG staff undergo thorough screening before they can deliver care.

Our clinical team includes AHPRA-registered nurses (both registered nurses and enrolled nurses) who maintain current professional registration and ongoing professional development. All care workers hold relevant qualifications and certifications for their roles.

Every team member has a current National Police Check, Working with Vulnerable People clearance, and is verified through Aged Care Worker Screening where applicable.

We are fully insured with comprehensive professional indemnity insurance, and we are a member of the Aged Care Provider Association (ACCPA), demonstrating our commitment to industry standards and continuous improvement.

Can I choose my own care workers?

GHCG prioritises continuity of care by assigning a dedicated care team to each client. We work closely with you to match carers based on your preferences, cultural background, language needs, and personal compatibility.

Our goal is for you to see the same familiar faces at every visit, building trust and a genuine relationship with your care team. Whilst we cannot guarantee a specific worker for every single visit (due to leave and availability), we maintain a small, consistent team assigned to you so that everyone who comes to your home already knows your care plan, your preferences, and your routine.

What languages do your staff speak?

We have a multilingual care team and are deeply committed to cultural competence. Our staff speak a variety of languages and understand the diverse cultural needs of the communities we serve across Sydney and NSW.

During your initial consultation, we discuss your language preferences and cultural needs so we can match you with a care team you feel comfortable with. We believe that care delivered in your own language, with respect for your traditions and customs, leads to better outcomes and a more positive experience for you and your family.

What areas do you service?

We currently service Sydney and surrounding areas of New South Wales, including Padstow, Bankstown, Canterbury, and the wider south-west and western Sydney regions.

How quickly can services start after approval?

We understand that when you need care, you need it promptly. Once you receive your Support at Home approval from Services Australia, we arrange a free consultation to discuss your needs and goals.

From there, we develop a personalised care plan with you, and services can begin shortly after — often within days of your care plan being finalised. The timeline depends on your specific needs and the availability of your matched care team, but we pride ourselves on responsive, efficient service commencement.

How does quarterly budgeting work under Support at Home?

Support at Home operates on a quarterly budgeting cycle managed by Services Australia. Your total annual funding allocation is divided into four quarterly amounts.

At the start of each quarter, we work with you to plan which services you will use and how your funding will be allocated across Clinical Care, Independence, and Everyday Living categories.

This quarterly structure gives you flexibility — if your needs change during the year, you can adjust your service mix each quarter. We help you make the most of each quarter's budget and ensure you are getting the services that matter most to you.

What are the three participant groups under Support at Home?

When Support at Home commenced on 1 November 2025, participants were grouped into three categories:

Grandfathered Participants — transitioned from an existing Home Care Package on that date, with their existing care arrangements largely preserved and funding classifications maintained.

Hybrid Participants — approved for Home Care Package funding during the transition period but had not yet commenced services, so they moved directly into the new Support at Home system with equivalent funding classifications.

New Participants — assessed and approved from 1 November 2025 onwards under the new Support at Home system and its 8 funding classifications.

Regardless of which group you belong to, GHCG can support you and help you make the most of your funding.

Is clinical care really 100% Government funded?

Yes. Under the Support at Home program, all Clinical Care services are fully funded by the Australian Government with no client co-contribution required.

This includes in-home nursing care by registered and enrolled nurses, wound care and dressing changes, medication administration and management, chronic disease and condition management, health monitoring and assessment, palliative and end-of-life care, and post-hospital recovery support.

You will not have any out-of-pocket costs for these clinical services. Co-contributions may only apply to services under the Independence and Everyday Living categories, and even then, there is a lifetime non-clinical co-contribution cap of $130,000 to protect you.
